How to Become a Contributor

The Living Well with Chronic Diseases web portal encourages submissions from organizations whose work is related to prevention and control of diabetes and is based or has a significant presence in the State of Washington.

To become a contributor and post your organization's events or resources on the Living Well site you must first request a user account. You can do that by clicking on the "Request a User Acount" link below and filling out the join form.

Request a User Account

After you fill out and submit the form you will receive an immediate confirmation that your application has been sucessfully submitted, followed by an email confirmation, user id, and instructions to set up a password.
