Leader Registry Links

If you are a CDSMP or Tomando Lay Leader or Master Trainer who wishes to sign up for the leader registry, or view the list of registered leaders, you must first become a contributor to the Living Well website. 

If you are already a contributor, please click on the "CDSMP/Tomando Leader Registry Form" link above to register yourself.  If you would like to view the list of registered leaders, please click the "Leader Registry" link above.

To become a contributor, follow the steps below:

• On the left side of the page is a “Log in” box with the last line in that box “Become a Contributor”. Click on “Become a contributor” and then click on “Request a User Account”.
• After you fill out and submit the  “Request a User Account” form, you will receive confirmation that your application has been successfully submitted. An email confirmation will follow, along with instructions for creating your own user id and password.
• Once you have created your user id and password and then log in as a contributor, you can return to the Professional Resources tab and then click on the CDSMP/Tomando Leader Registry Form. Fill out the Leader Registry form and submit.  Within a few days your information will be posted to the Leader Registry.
