Are you interested in healthy lifestyles and helping others to improve their health?
Do you enjoy working with people?
Do you have a long term health condition or do you live with or help care for someone who does (note: this is not required)?
Are you interested in being part of an exciting world-wide program?
Can you give a few hours a week, for 12 weeks over the next year?
If so, be a Living Well with Chronic Conditions Workshop Leader!
What is the Living Well Workshop? A Chronic Disease Self-Management six-week program developed at Stanford University for people who have, or care for someone who has, a chronic health condition such as diabetes, heart failure, arthritis, depression, etc. Workshops meet once a week for 2.5 hours.
Physicians of Southwest Washington is working in cooperation with Pierce County Aging and Long Term Care to provide this training for anyone interested in participating.
Persons living with chronic health conditions are especially encouraged to apply.
