Living Well with Chronic Conditions

This six week workshop teaches practical skills for living a healthy life for participants with an ongoing condition such as asthma, diabetes, high blood pressure, heart disease, obesity, arthritis or other conditions. Developed by Stanford University’s Patient Education Department, the workshop is designed to help people learn to manage their chronic conditions and improve the quality of their lives. Family members, friends and caregivers can also participate. By participating in the six week workshop, people increase their confidence and motivation needed to manage the challenges of living with a chronic health condition.
