My Health Matters-Chronic Disease Self-Management Workshop

CHOICE Regional Health Network is pleased to announce the MyHealth Matters Chronic Disease Self-Management (CDSM) Workshop, taking place July 27-August 31, 2011. This Stanford University program provides those living with chronic disease tools to help manage their condition(s). This workshop has a participatory and supportive format that is designed to help individuals dealing with a range of conditions including but not limited to asthma, diabetes, arthritis, depression or chronic pain. It is facilitated by two Stanford-certified leaders, covering the following subjects: • Techniques to deal with problems such as frustration, fatigue, pain and isolation; • Appropriate exercise for maintaining and improving strength, flexibility, and endurance; • Appropriate use of medications; • Communicating effectively with family, friends, and health professionals; • Nutrition; and • How to evaluate new treatments. There is no cost to attend this workshop.
