The Positive Self Worth program has been specifically developed to help you learn to take charge of your health care challenges and improve your quality of life. Based on the highly successful Stanford University Chronic Disease Self Management Program, Positive Self Worth can help you create a more positive outlook – and feel better about yourself – by providing you the tools you can use to live more fully with a chronic condition.
