Positive Self Worth (Chronic Disease Self Management Program)
This six week workshop teaches practical skills for living a healthy life for participants with an ongoing condition such as asthma, diabetes, high blood pressure, heart disease, obesity, or arthritis. Developed by Stanford University’s Patient Education Department, the workshop is designed to help people learn to manage their chronic conditions and improve the quality of their lives. Family members, friends and caregivers can also participate. By participating in the six week workshop, people increase their confidence and motivation needed to manage the challenges of living with a chronic health condition. Three ways to register either : 1) call Physicians of SW WA at 360-786-8690 ext 127 and ask for CDSMP information 2) Call Kate Greene at South Sound Senior Services 360-586-6181 ext 111, or 3) Request a referral from your case manager if COPES eligible
