Chronic Disease Self-Management Education

Better Choices, Better Health, is an online workshop given entirely on the Internet where people with different chronic diseases attend together.  There are no requirements that participants log in at the same time.

It is the process in which the online workshop is presented that makes it unique.  Sessions are highly participative through internal messaging and online discussion boards, where mutual support and success builds the participants' confidence in their ability to manage their health and maintain active fulfilling lives.  Participants are asked to log in at their convenience 2-3 times per week for a total of about 2 hours per week. 

Subjects include:

  • Techniques to deal with problems such as frustration, fatigue, pain, and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Healthy eating
  • Decision making

The Chronic Pain Self-Management Program is a small-group workshop where people experiencing chronic pain attend together.

Subjects include:

  • Techniques to deal with problems such as frustration, fatigue, isolation, and poor sleep
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • Pacing activity and rest
  • How to evaluate new treatments

The Diabetes Self-Management Program is a small-group workshop where people with Type 2 Diabetes attend together. 

Subjects include:

  • Techniques to deal with the symptoms of Diabetes, such as fatigue, pain, hyper/hypoglycemia, and stress
  • Techniques to deal with problems such as depression, anger, fear, and frustration
  • Appropriate exercise for maintaining and improving strength and endurance
  • Appropriate use of medications
  • Working more effectively with health care providers
  • Healthy eating

Program de Manejo Personal de la Diabetes is a small-group workshop conducted in Spanish, with resources in Spanish, where people with Type 2 Diabetes attend together. 

Subjects include:

  • Techniques to deal with the symptoms of Diabetes, such as fatigue, pain, hyper/hypoglycemia, and stress
  • Techniques to deal with problems such as depression, anger, fear, and frustration
  • Appropriate exercise for maintaining and improving strength and endurance
  • Appropriate use of medications
  • Working more effectively with health care providers
  • Healthy eating

Tomando Control de su Salud is a small-group workshop conducted in Spanish, with resources in Spanish, where people with different chronic diseases attend together.  The topics are similar to the Chronic Disease Self-Management Program (CDSMP), but they are presented in ways that are culturally appropriate.

Subjects include:

  • Managing depression
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Healthy eating
  • How to evaluate new treatments
  • Relaxation techniques
  • Appropriate use of the healthcare system
  • Better breathing
